Quilt/Garment Competition Entry Agreement: I enter my quilts(s) and/or garment(s) and acknowledge that MQX, its organizers, staff, families and volunteers will do everything possible to secure the entries while in MQX’s possession. By signing this authorization, I agree that I will not hold any of these parties liable for loss, damage while at MQX or during transport to/from MQX. I further agree that it is up to me to insure my entries and release MQX from any liability exceeding $500. I agree that MQX is not liable for any payment due to loss or damage of any kind or under any circumstances. I hereby certify that all information listed on the Quilt or Garment Competition Entry Form is true. I fully understand that my entry may be disqualified if any statements made are inaccurate. I give permission for my entry to be photographed for MQX publicity purposes, the MQX Photo CD and any photography needs relating to this event.
